  1. Construction flaws: The use of cast-iron rivets caused the hull to be weaker.

    Design Flaws: The design of the bulkheads allowed water to sleep over the top. They were also not designed to have more than one punctured.

    Crew Mistakes: What the top person said is valid, however the lookouts not having binoculars also contributed. The fact that the ship had to turn at the last minute made the opening in the hull really large.

    There are more reasons as to why it was so deadly as well that design flaws contributed to.
